All potential students/applicants must complete a DODD application, meet the basic requirements, and provide all mandatory documents to the Nurse Educator in order to be considered for enrollment into any of the Medication Certification sessions.

Mandatory requirements before acceptance include:

1. High School Diploma, its equivalent, or college transcript
2. Completed DODD Application Form
3. Valid Photo ID (Driver’s License/ State ID/ Passport/ Permanent Resident Card)
4. Enrollment Agreement – Review and Sign
5. Valid Email

Medina Nursing Services LLC reserves the right to reject applicants if the admission requirements and procedures are not successfully completed.

Students can pay on the day of class.

CERTIFICATION REQUIREMENTS

Medication administration training is measured in class hours. The required hours for each program must be completed. In order to receive certification, each student must have a minimum knowledge base by scoring 80% or better on a closed-book, multiple-choice test.

Class cancellation policy

In the event of illness of the instructor, severe weather, or training facility issue you will be contacted by phone at the number on file with RN Instructor.

Students will be notified by phone or e-mail regarding class make-up dates when class is canceled by the instructor, please make sure you update your contact information if it has changed since you were registered and approved for the training.

Cancellation/ Refund Policy

Three business days before the start of the class, a 100% refund of all registration fees is permitted. Refunds must be requested by 5 pm on the third business day. The request can be made by email or fax. RN instructor or office manager will contact the individual making the request. Refunds are issued within 15 days of the cancellation date and will be sent via US mail.
